I declare that the player is medically fit to take part in football training and that all known medical conditions have been disclosed to the academy.As parent/guardian I consent to this registration, to the academy processing the player's data for development purposes, and to academy image use in line with the safeguarding policy.

Total initial payment due: ₦50,000 — registration, jersey and first month of training. Monthly training continues at ₦10,000 per month.
